This is how you exercise any of the rights in the Privacy Policy and the Consumer Health Data Privacy Policy. Fill this in and we will have a record of it, with the time it arrived and the date our answer is due. You can also email privacy@trylifelong.com instead — that route is equally valid and always will be.
what we commit to
- We acknowledge within 10 business days.
- We complete verified requests within 45 days, and sooner where the law where you live requires it — in Australia, within 30 days. Where the law allows us one further 45-day extension and we need it, we will take it and tell you why.
- It is free. Requests about consumer health data are free and unconditional without exception.
- You will never get a worse service for asking.
the request
about verifying who you are
We may need to confirm your identity before we act, and for health data we will ask for enough to be confident — handing your records to an impostor is the worse failure. What that means in practice depends on what you have asked for: confirming control of the email address on the account is often enough, and we ask for more only where the request would disclose or destroy health information.
Anything you give us to verify yourself is used to verify you and then deleted. It does not go onto your account, it is not used to enrich anything we hold about you, and it is not kept as a record that you made a request.
Please do not send identity documents through the form above. If we need them we will write to you and say exactly what we need and how to send it.
what we store about the request itself
The name and email you give us, what you asked for, anything you wrote in the detail box, the country and region you told us, whether you are asking for yourself or someone else, and the timestamps — when it arrived, when we acknowledged it, when we completed it. We keep that record because a promise about deadlines is only meaningful if the deadlines are written down, and because several of these laws require us to be able to show how we handled a request.
if you are unhappy with our answer
You can appeal. Reply to our decision, or email privacy@trylifelong.com with "Appeal" in the subject line, and we will respond within 45 days with our reasoning. If we deny the appeal you may complain to your state Attorney General, your supervisory authority in the EEA or UK, or the Office of the Australian Information Commissioner. See Privacy Policy §13.
